Now to your question: I would say no. He could do a design, present it and - when your client is satisfied with it - create an install package that you can upload as an extension to the web site. You then have to apply the skin (which could be a bit challenging when you have a large website and there is too much layout definitions on the page and/or the module containers, but is not a real problem), and that's it.

There is absolutely no need for a designer to gain administrative permissions, not for the work and not for installing. The great thing with DNN is that things like design, content and business logic are completely separated, which makes it highly extensible.
