The feeback module form is not displaying the label for the Postal Code field.  The text box is there, but not the label.

This problem occurred after we upgraded DNN from 05.06.00 to 06.01.04 (professional) and upgraded the Feedback module to 06.00.00.

Emulated same error using community on my desktop 06.01.05  -- Problem with feedback version 06.00.00 and 06.00.00 but not earlier Feedback module version 05.01.00 (its displays the label okay).

I can confirm that this is an issue in Feedback 06.00.00 and is due to a typographical error in the spelling of the ResourceKey attribute value for the Postal Code field label in the file Feedback.ascx.

I will log it as an issue in the project's Codeplex issue tracker. In the meantime, it can easily be fixed by editing the file DesktopModules/Feedback/Feedback.ascx in any text editor:

1. Find the line:

<asp:label id="plPostalCode" runat="server" associatedcontrolid="txtPostalCode" resourcekey="plPostCode" />

2. Change resourcekey="plPostCode" to resourcekey="plPostalCode"

3. Save the edited file and if necessary re-upload it to the same site folder on your shared hosting provider.
