I don't have an icon for inserting images in my HTML module.  Is there some setting I need to enable?  I know when I configure other RTF editors like FreeTextBox, that I have to enable/disable different buttons.  I can't find anything about this.  Thanks.

check out the document manager menu (paper clip icon).
If you prefer a toolbar with "insert image" icon, have a look at free CKEditor provider (dnnckeditor.codeplex.com)
