Yes it's very strange. I checked all files and all seems to be there.
I tried to copy all files from the install package in desktopmodules/admin/filemanager but it changes nothing.

It seems it doesn't find the instance for the filemanager. I checked the dotnetnuke.dll and it's the latest one.
I have no more idea from now.

please check file permissions as well and make sure, they are not "blocked" (because downloaded from the internet - see first tab of file properties)

I checked file permissions, apply again the good ones.
I also tried to modify the application pool 2.0->4.0 but no change.

I tried to comment this code in default.aspx.cs:
//var fileInfo = FileManager.Instance.GetFile(PortalSettings.PortalId, PortalSettings.BackgroundFile);
//var url = FileManager.Instance.GetUrl(fileInfo);

// ((HtmlGenericControl)FindControl("Body")).Attributes["style"] = string.Concat("background-image: url('", url, "')");

And the page load but with many errors, no menu ...
I tried to modify the portalsettings but it crashed.
Every time there is an access to FileManager.Instance, it returns nothing so cannot work.I think it's a web.config problem but I can't findout where the provider could be missing.

I saw a similar problem upgrading a site, what I found in that case is the problem was the logo associated with the portal. I went in and set the LogoFile to null and I was able to use the site and re-assign the logo again. I don't know if this is your issue, but its worth a shot (I assume the root of the problem is the new file provider not migrating something properly on upgrade).
