I would definitely say that your issues are not normal.  There have been many successful upgrades to 9.1.1 .  Unfortunately, we haven't been able to work with anyone that has had these issues.  I'm replying mainly so that if anyone else sees the post and has these problems, please email me: will -at- dnnsoftware.com.  We want to understand why this type of situation does occur for some users.

Managed to resolve our issue by adding the following to web.config

I have been looking through these forums for issues similar to mine and this was the closest so will post here.

I have a DNN site that started at version 6.2 and has been upgraded over the years to 7 and then 8.  I've just tried a v9.1.1 upgrade on it and it went smoothly until anyone non-Host tried to edit the site - they weren't seeing the Edit pencil icon in Personabar.  There was also no Content menu from where to change Pages.  The other two (Manage and Settings) were working correctly.  I reset permissions on the page and then removed their Adminstrators role and added it back in - but nothing fixed it. 
I  then found a mention to the PeronaBarMenuPermission table and had a look in there.  The site is actually made up of 4 portals with the main one being portal = 0 and having two languages.  I noticed that the PeronaBarMenuPermissions table only had a single entry each for MenuId = 1 and MenuId = 5.  These are the Content menu and the Edit (ie pencil) menu.   The PortalId for both of these was 2 (one of the other portals in the install).  So I manually added a row for Portal 0 and Cleared Cache and now the Admins can edit! 
I guess there was some issue during the upgrade however I didn't receive an error at the time.  I've upgraded several other DNN sites to v9 without this happening so it must be something specific to the site.
