I am trying to find a module that would work similar to the Feedback module, but that would allow flexibility in defining more fields in a form and controlling the sequencing/positioning of the fields in the form and then store the results but also send them in emails simialr to the feedback module.  Can the Feedback module do that (perhaps with settings I am not yet aware of), or is there another module that would be more suitable for this.

The current version of the feedback module does not offer customization (version 4.4.3) even though the newer 5.0.0 does offer the customization you're looking for. 5.0 has not yet been released though. There are a couple of commercial modules out there that would probably meet your needs - take a look on snowcovered.com or DotNetNuke MarketPlace and you should get some hits from the search there.

Thanks for the info Sanjay.  I looked in the Forge to find out the status of Feedback 5.0 and noticed that there wew notices that it was available last year.  What happened to it.  Do you have any idea of when it will be available?  Is it possible to get it in its current state?
