I changed the Attributes definition from single to multiple (on the header.xml as detailed on other posts) and when I submit a record I get an error: "Unable to cast object of type 'System.Web.UI.WebControls.RadioButtonList' to type 'System.Web.UI.WebControls.CheckBoxList'"

Does this mean I have to delete all the items I have and create another instance of the module or is this a bug?

yes it's a bug, see the following thread
http://www.dotnetnuke.com/Community/ForumsDotNetNuke/tabid/795/forumid/54/threadid/95485/scope/posts/Default.aspx

at the end of the thread is the code you would have to change to allow radio buttons for categories and/or attributes
