Does anyone know where I can find a tutorial on creating a questionnaire with the form and list module?
Can a questionnaire be easily created with this module?

if you just want to collect data, you may use F&L, using a form view and on another page a (admin visible only) instance with results, you may download and evaluate. if you need a survey with single/multiple choice options only, you may use Survey module instead.

Thank you. I need more 'advanced' options (not only option buttons and check boxes but also drop down boxes and text boxes), so I'll have a look at the F&L. I do need to save it in my DNN database. I'm guessing that this is possible. Am I correct?

Given my requirements, would you advise the F&L module, the free Helferlein Forms module (from www.helferlein.com) or building my own module?

FnL will save the data in the database - but serialized in a single table/column. I am not sure, how Helferlein.com saves its values, another option might be using one of the commercial 3rd party options, e.g. DataSprings FormMaster. If you have some special requirements (dynamic DLL, values depending on replies to other questions), creating your own module might be easier.
