I have located the problem.  It seems that some users have passwords that make System Administrators very, very sad.  I "fixed" (ugh) the line in the web.config that required decent passwords and we were able to login several of the users.

can you please share what needs to be changed in web.config. I am facing exactly the very same issue on my installation after upgrading to 5.1.1.

 Sid, you mean where some users can log in and some can't? How long are their passwords? By default DNN expects 7 characters for the password. Automatic logins bypass this but manual logins use the DNN login process and will get caught.  If you search your web.config for minRequiredPasswordLength you can set it to the length required by your domain.

Just ran into this same issue.  Some AD users could log in, some couldn't.  Changed minRequiredPasswordLength="1" in the web.config, now all AD users can log in.  Thanks!
