A number of them.

For an easy-to-use module with good functionality see Form Master. For more complexity with workflow and greater features see Enterprise Forms. Others to roll your own exist, such as XMod. There are others besides these that I've seen mentioned but know nothing about; search the forums, or google "Dotnetnuke forms module" perhaps.

You can pretty much do anything with alittle creativity.


Do you use Oracle?  Then try GoldFish.Net

In my search for custom form modules that save records to an SQL db and allows users to query these data submitted by other users, I looked at Dynamic Forms (all these are searchable at snowcovered), Form Master, XMod, ListX, Indoo SQL Grid, and Enterprise Forms.  I received some great advice in a recent post in the forums explaining the benefits and limitations of each....but these seem to be the most popular and powerful form builders I came across.

I'm still undecided, so let me know if you pick one and run with it...let me know how you make out.

We have been using Enterprise Forms for our customers since its early days and believe me it is powerful product.   The flexibility in its workflow engine is something I have not seen in any other modules (forms or non-forms) availabe on the DNN market.  This was the sole reason we selected Enterprise Forms over other products.  Building a form with DB backend and search capability is something pretty much all these form builders should have.

 

Frankly I can't comment on other form builders as I have yet to have the need to use them.  At the end of the day it comes down to how much you want to pay and what you want  to do with your form.  If you just want to build an enhanced version of a feedback form then I think any of the listed form builders suffice, but if you require more flexibility and be able to achieve complex forms then XMod, ListX and Enterprise Forms may be your best option.  Of course flexibility comes with a price and that is a learning curve.
