I'm pretty sure this has been addressed to the considerable amount of hairloss from the DNN community overall,  so the next release has been focussed on addressing this maddening problem!!

So perhaps a cap, hat, or some form of head cover is required for the next few days, while they get the build ready for official release!

Have you guys upgraded to 4.0.3?  Has it corrected this issue?

I have had this problem before, we were running 4 worker processes. I found that running just 1 worker process corrected the problem.

Thanks for that gaz2sg,

I'm running multiple worker processes as well, and have just upgraded from 3.2.2 to 4.0.2 to see if it fixed the problem.  It didn't.  I'm now going to go in an shove all my DNN stuff into one worker process and see if it fixes the problem.  Stay tuned!

I would give 4.0.3 a run. I hear that fixes the problem but have not upgraded any of my installations yet.
