Yes, I gave the USER Full Control to the DNN directory.

I am not sure what you mean by this?  Sorry for being a Newb

For kicks, I made up a backup copy of the DNN 4.9.4 directory and database to another one and proceeded to upgrade the DNN to 5.1.1.
I wonder if this problem will follow.  Ill let you know.

I upgraded to 5.1.1 with the new AD module too and the error still persists.  This is frustrating for sure.

 With SQLExpress and possibly the full version of SQL there's two ways you can create a database. One is by attaching a *.mdf file to it which is how it looks like you've done it and the other is the pre-SQL2005 way of manually creating the database. Why you're getting this error is a mystery to me.

Well, after going back and forth from 4.9.4 to 5.1.1, I decided to try 4.9.4 again and start from scratch.   I accidently forgot to uncomment the line for the impersonation and the DNN was working with AD without a problem.  It worked without any errors for day and half.  So, I decided to add the impersonation line with Username and Password this morning and within an hour, I got the Unhandled Error message.  Looks like its the impersonation which causes this. 

Do I really need the impersonation line for AD to run?

If it's working without it with no problems then no you don't.
