Then the user had request the e-mail notification on the footer of the forums site, or on a footer of a thread.

I do not have any such setting enabled. Yet I get emails for every post and thread on the entire forum. I even get emails when a new thread needs to be approved by a moderator, and I am not a moderator! Can there be some corrupted setting in the database that is not being displayed in the UI? Where is this information stored in the forums module table?

Vin, in this post I show how you can see subscribed users: http://www.dotnetnuke.com/Community/F.... The two queries i show in there will show you who is subscribed to a forum or a thread (2 separate queries). If you download the Beta 4.6, this actually has a user interface that executes these and shows the results in a datagrid. There is also a user interface to show what emails are being queued up specific to the forum and who they are being sent to. 

As for the other things to check, the notification forum type previously mentioned is important (its the only reason I can see this happening if things are as you say they are). A notification forum will be stored in the Forum_Forums table, and will have the ForumType colum set to 1. You should also check to see if the queue is enabled or disabled (Host -> Scheduler, locate the DotNetNuke.Modules.Forum.StartEmailQueuedTask one). The only other setting around email is also visible in the UI, via Email section of the Forum Admin. Under Email Settings, you will see "Enable Notifications" and also an "Enable Email Queue" that can tell you more.

I ran those queries, no results for any of the forums. All Forum_Forums.ForumType records are value '0'.

I think there may be some data corruption going on. We previously had a much older version of the Forums installed (version 3 I think?) and I think some of the old data is still present. This is only one example of strange behavior.

Because of big changes in the past, the upgrade probably dropped those who were subscribed so my guess is that might be your issue.
