Yep, max worker processes is set to 1.

Any other thoughts?

if you directly access a newly uploaded image by path, do you get it displayed correctly, despite being logged on?

No.  I used a new image uploaded the image map editor, and the result was the same.  The new image appears in Edit Mode, but once you close Edit Mode the old image is still there.

no, I mean, are you able to access the image by using it's direct URL, even if unauthenticated (rightclick in page to get the URL of the image).
