Thanks. I switched to Ribbonbar and that cleared some issues but I am still seeing the same authentication issue from the first post in this thread - but when trying to upload anything from profile pages. If a user logs in, goes to their profile page and tries to change their account picture or attach a picture or file to the activity feed then they get the DNNAPI security prompt. Also happens when interacting with group pages.

File manager works fine and the HTML editor file manager works.

IIS7 using NetworkServices (tried various AppPool accounts - same result). Folder permissions look fine to me.

It works as expected with Firefox, but not with IE 7, 8, 9 or 10.

I've searched for two days for a solution but have been unable to find one. Has anyone else experienced this or have any ideas for a possible solution?

My issue seems to be resolved after updating to 7.0.3.
Thanks!

With the help of @erikVB we solved it by changing the web.config:
<system.webserver>
< modules runallmanagedmodulesforallrequests="true" >... ...

we added the property: runAllManagedModulesForAllRequests="true" , it shouldn't do anything in iis7, but it did...

Well, I experienced the same issue. I'll try to add more information. I have a couple of sites in DNN6 which I updated to DNN7 without problem. Control bar works well. But when I made a new site this error occurs! So I got into this post. Then I compared web.config files from a DNN6 updated site with a DNN7 new one and I've seen a difference that makes me success.

<modules>
...
<remove name="FormsAuthentication" />
<add name="FormsAuthentication" type="System.Web.Security.FormsAuthenticationModule" preCondition="" />
...
</modules>

I don't know if it will work for everyone but it works in my case, a least inserting modules or editing pages, that was impossible before.

This has happened to me too. I was upgrading from 6.2.3 to 6.2.6 and did my upgrade by copying all the files from the upgrade on top of the files in my application folder. The upgraded proceeded fine and all generally looks and works well except for the authentication pop-up problem.

 I can login to the site and all the page functions seem to be ok, but every page I navigate to is preceded with the "authentication required" pop-up. If I cancel the pop-up all still works fine. Similar to #Jason I don't seem to encounter this problem when using FireFox, but IE and Chrome both throw the pop-up.

I was assuming that it was some sort of file permission error, but haven't been able to figure it out. I have done upgrades before using the same process and did not run into this problem.

Any obvious reasons this would happen? I am using IIS v6 and windows server 2003 r2. I have an older dnn application on the same server experiencing no problems and previous to the upgrade this application was running fine.
