Well.. after first wasting even more time and money on Snowcovered on a module called Spascoe Complete Feedback Designer, I finally gave in and bought Xmod. It's not a ready-to-go solution, but if you're planning on using forms in other forms  on your sites in future, you probably have to get a module like it anyway. I pieced together a form equivalent to the core feedback form, but with the categories and tight layout I wanted, within a couple of hours by going through the tutorial. And wonder of wonders... it's a DNN module with documentation!

Another alternative would be to just grab a ready-made script somewhere and paste it in as code.

This is the latest version I have written on creating Feedback Categories.

  1. Navigate to the Host > Lists page.
  2. Click Add List.
  3. In the Entry Name field, enter Feedback_Categories.
  4. In the Entry Text, enter the first category name. E.g. Marketing.
  5. In the Entry Value field, enter a value for the category name. E.g. MAR, or 1.
  6. Click Save.
  7. Repeat Steps 2-6 to add additional categories to the list.

Not sure if you're still looking, but I needed a replacement and ended up writing it myself..  You can download it from here:
http://www.orizonti.com/DotNetNuke/NukeFeedbackModule/tabid/2146/Default.aspx

It's similiar to the Enhanced Feedback, but I took it a little bit further with allowing the selectable subjects to go to different email addresses..
