My AD Admins are concerned about updating user permissions in two places. Active Directory and DNN.  Is there a version that automatically updates/Refreshes the user roles / groups? How long does it take to refresh?

Out of the box in DNN 4.5.5 and below the roles are only synchronized the first time a user logs in. With the DNN AD Fix applied (http://dnn.gmss.org) it'll happen everytime a user logs in and when DNN 4.6.0 is released it'll happen everytime.

I also noticed that if I add a user to a role that the user doesn't belong in Activedirectory, the user isn't subsequently removed. IE a user changes departments. Shouldn't the user be removed from the old role and added to a new one?

Yes he should but again that's because the default AD configuration only synchronizes the roles the very first time a user logs in.  This has been changed for 4.6.0
