I'm going to hijack this thread a little bit.  :-)

Instead of doing all this work trying to make a DotNetNuke website run on a provider who obviously doesn't care about DotNetNuke, why don't you send us a copy of your site and let us migrate it to our hosting environment for you?  We have a very seamless process for migrating DotNetNuke and I'm sure we can rid you of many of these headaches.

Thanks everyone for your assistance - managed to start all over again, uploading the source files to the remote server, amending the web.config and starting from scratch :-)

Although it means I have to do a little repeat leg work, the pain with dealing with the hosting provider is now gone!
